A 504 Plan is a formal plan developed to ensure that a student with a disability has equal access to education. It provides accommodations and support in the general education setting—such as extended time on tests, preferential seating, or help with organization—to remove barriers to learning.
Unlike an IEP, a 504 Plan can follow a student beyond K–12 schooling, including into college or vocational programs. As an advocate, support is provided by helping families understand eligibility, guiding them through the 504 process, and ensuring that the accommodations are appropriate, clearly written, and consistently implemented.
Help the Education Department understand the needs and desires of the community by completing this form for EACH member of your family!
